The Australian Ovarian Cancer Study is a collaborative research program between clinicians, scientists, patients and advocacy groups aimed at improving the prevention, diagnosis, and treatment of ovarian cancer. Initiated in 2001 by researchers at the Peter MacCallum Cancer Centre, University of Melbourne, Queensland Institute of Medical Research and Westmead Hospital, the first goal of the study was to create a uniquely powerful resource for ovarian cancer research comprised of bio-specimens, clinical and epidemiological data. With an initial goal of developing a cohort of a thousand women with either borderline or invasive primary ovarian cancer, by 2006 a total of 2456 women from all Australian states had consented to take part in the study. Of these, 1859 women were eligible cases with either borderline (22%) or invasive disease (78%). Clinical follow up information is collected prospectively through a national research nurse network and to date less than 7% of women have been lost to follow up.

Detailed epidemiological data have been collected from cases and demographically matched control women (1073 enrolled), allowing us to refine information about known environmental risk factors, and understand how environmental and genetic risk for ovarian cancer may intersect and guide prevention strategies.

AOCS is engaged in a widespread genomic analysis of cancer bio-specimens, with the goals of comprehensively mapping gene expression and DNA copy number variation in tumour samples and relating this information to response to treatment. A major goal of the study is to identify genetic and biochemical changes in patient’s cancers that dictate responsiveness to chemotherapy and overall patient survival.

Mutations in several genes, including BRCA1 and BRCA2, are known to substantially increase a women’s risk of primary epithelial ovarian cancer. Recent data from North America indicate that these mutations are more common than previously thought. AOCS represents a unique population-based cohort where such findings can be validated and results used to inform guidelines for offering genetic testing to women with invasive ovarian cancer and their family members. AOCS researchers are also attempting to identify other genetic variants that may contribute to the development of ovarian cancer through our own studies and as an active member of the international Ovarian Cancer Association Consortium (OCAC).

AOCS is an international resource, which researchers can apply to access through the AOCS management committee. Applications are assessed on the basis of their scientific merit, whether they represent an appropriate use of scare resources (such as bio-specimens) and whether they have gained approval from a properly constituted institutional human research ethics committee.

AOCS represents an outstanding resource to address clinically important questions about the causes and treatment of ovarian cancer, made possible through the collaborative spirit of clinicians, nurses, pathologists and scientists throughout Australia and through the willingness of thousands of women to participate in this vital program.
